RN Liaison/Customer Advocate

North Mississippi Health Services

I came to NMHS in 2004 and worked as a route nurse providing direct patient care in Itawamaba and Tishomingo Counties in North East Mississippi. I was promoted to Liaison /Customer Advocate for this region. I now work out of NMMC Iuka and Iuka Medical clinic serving as liaison for 15 physicians and nurse practitioners in this region. As a liaison, I set up patients on Home Health and Hospice, provide public relations in this region, and handle communications to and from the physicians for the patients we serve.

Heart to Heart Hospice

At Heart to Heart Hospice, I served in a variety of roles. Starting out as a route nurse providing direct patient care for patients in the multi-county region served. I was later promoted to case manager, then director for the Belmont, MS and Red Bay,AL branches, in which I was in charge of all clinical staff to include nursing, aides, social workers and chaplains. Additional skills achieved at Heart to Heart include teaching aide classes and certification for hospice aides, hired/fired clinical staff, and provided monthly inservice meetings for clinical staff.

Tishomingo Manor Nursing Home

At Tishomingo Manor, I worked in a variety of roles, beginning as a medication nurse on 3-11 shift. I was promoted to medication nurse on dayshift, then became charge nurse. In 1998, I took the position of Medicare Case Manager, in which I managed the care of all patients coming into the skilled Medicare bed program. This included direct patient care with each  patient, worked directly with the therapy department, and managed the PPS system for payment reimbursement. I was TB certified, CPR certified, and trained in the MDS forms required for Medicare.

My name is Sarah Hammock. I have been married for 15 years and I am the proud mother of 2 children. Haley is 11 years old and in 6th grade. Brady is 4 years old and in preschool.  I am active in our local church, Forrest Grove Baptist Church. I am a Sunday school teacher and work with the youth of the church.  I have been a Registered Nurse for the last 13 years and I am currently pursuing my Bachelor's Degree in nursing. My long term goal is to get my Master's Degree in Nursing and purse becoming a Nurse Practitioner.
